Persons with Disabilities

  • All placards and plates issued or renewed, require a New Application form, completed and signed by a medical professional
  • There is no fee for placards
  • There is no grace period for placards
  • Anyone other than the applicant may witness the applicant's ID information on the form
  • Completed forms may be faxed, mailed or processed in person
  • If the form is processed in person, the applicant must be present with photo ID or representative must show Power of Attorney to receive the placard over the counter.
  • All other situations will result in the placard being mailed to the individual named on the application
